Aubrey Plaza and Jeff Baena were separated for four months before he tragically died, according to a new report released by the Los Angeles County Department of Medical Examiner.

“Jeffrey Baena was separated from his wife [redacted] since September 2024 when she moved to New York,” the report, released on Wednesday, states.

“The descendant had a phone conversation with his wife last night.”

Authorities spoke to Plaza on the phone on Jan. 3, and she “confirmed” the separation, the report adds.

“In October 2024, Baena made concerning remarks to [redacted], which prompted her to call a friend to perform a welfare check on her husband,” Lieutenant Kelsey Weber states.

“Baena had been attending therapy since the event,” she added. The officer also noted that the screenwriter did not have any “previous suicide attempts” but had “difficulty sleeping.”

“[Redacted] last knew her husband to be alive when he sent her a text message on 01/03/2025 at 1036 hours (unverified Eastern Standard Time),” the report concludes.

Page Six has reached out to Plaza’s reps for comment on the report but did not immediately hear back.

Baena died by suicide on Jan. 3. He was 47 years old. The filmmaker’s official cause and manner of death was also released in Wednesday’s report, which included a detailed description of how he passed.

It was unknown to the public that Plaza and Baena had parted ways.

In 2021, the “White Lotus” actress revealed that she and the “Horse Girl” writer had secretly tied the knot a year prior.

“So proud of my darling husband @jeffbaena for dreaming up another film that takes us to italia to cause some more trouble 😈,” she wrote about their new indie comedy project, “Spin Me Round,” at the time.

Plaza and Baena began dating in 2010 and then worked together on multiple projects like 2014’s “Life After Beth” and 2017’s “The Little Hours.” They were very private and rarely posted about their relationship on social media.

Plaza said in January that Baena’s death was an “unimaginable tragedy.”

“We are deeply grateful to everyone who has offered support,” the “Agatha All Along” star, along with the late director’s family, said in a statement to Page Six at the time.

“Please respect our privacy during this time.”
