By 2017, Cowell had already begun using Botox and fillers, as indicated by the additional volume in his cheeks and lower face. “This usually occurs when fillers are used to lift sagging skin, rather than opting for surgical removal of excess skin,” Dr. Richland clarified. “Such an approach can lead to a ‘pillow-face’ appearance, inadvertently making someone look more fatigued due to the filler’s weight pulling down the tissues over time.”

According to Dr. Richland, Cowell continued to utilize Botox and fillers up to 2024. “His forehead and glabella are remarkably smooth for a man his age,” he noted. While this might be the desired outcome, it has its downsides. “Over-paralyzing the forehead can cause the brow to lose its natural arch and sag, which makes the eyes appear heavier and more crowded,” Dr. Richland elaborated.
