Troye Sivan Speaks Out After TikToker Gave Unsolicited Plastic Surgery Advice

Troye Sivan is taking control of the conversation after a TikTok physician publicly critiqued his looks and suggested he undergo cosmetic enhancements.

The 30-year-old musician recently shared his thoughts on aging, self-perception, and societal pressures to look forever youthful in a Substack post dated Wednesday, January 21. In it, he expressed frustration over the uninvited plastic surgery advice but also acknowledged receiving a “thoughtful” apology from the doctor.

“I constantly fluctuate between feeling like I’m aging gracefully and getting more ‘attractive’ with time, and then feeling like a close relative to Gollum with a pop career,” Sivan began. “I feel worn out, somehow managing to be both underweight and overweight simultaneously. At 30, I’ve battled with body image issues throughout my life, much like many others.”

Sivan discussed the relentless pressure to meet a shrinking standard of beauty, noting, “Historically, I’ve been known for being very thin, and I’m not as thin anymore. I’ve also been labeled a ‘twink’—I’m still the top Google image result for ‘twink’—but I don’t quite fit that image anymore. Oy vey.”

Often the target of online scrutiny, the “One Of Your Girls” singer shared that he typically has two reactions to criticism. The first being, “I am deeply body positive and believe in the beauty of all bodies,” he explained. “I’m thankful for mine (and yours, beautiful), and for its ability to let me live freely, without pain or illness. Aging is cool too.”

Sivan said he heard those words of wisdom “from a person at the very top of the fashion world,” quoting the phrase, “stay ugly” while clarifying that it essentially meant “don’t f*** with your face.”

Adding to that point, the social media star said he’s proud of the recent efforts he’s made to improve his overall well-being, including going to the gym and sticking to a nutrition plan since December 2025.

“I’ve been getting bigger — still lean, but more muscley, defined, and toned. It’s only been 6 weeks and sometimes I’ll run my hands over my chest and feel like it belongs to someone else,” he shared.

Sivan noted that on the other hand, he’s often criticized himself and also dabbled with the idea of nips and tweaks, while pointing they can still be “dangerous” and turn out “bad” even after a massive investment. The star admitted that he was previously quoted $3,000 during a consultation about getting a fat transfer to the under eyes.

“I really don’t want that frozen look, but I do notice my ‘elevens’ are starting to show even when i’m not frowning. I frown all the time because I have light blue eyes that are very sensitive to sun,” he continued. “What good is money and modern medicine if not to fix all of these flaws that this random sicko f***o plastic surgeon told me I have in an instagram reel?”

Sivan vented about red carpet photos turning out less than desirable, confessing that “I’m too embarrassed to pose the way I think people who look good on red carpets pose.”

“Hearing this unsolicited medical advice given publicly by a doctor I do not know pushed me toward Prong #2 for a fair few days,” he continued. “My all-knowing, eternally and deeply evil algorithm saw the opportunity in this moment of vulnerability, and pulled every lever and dialled every knob to 1000. I saw video after video of deep plane face lift recoveries, and ads for unapproved GLP-1 meds that now come in pill form (I weigh 59kg/130lbs at the moment).”

Sivan concluded his post with a hopeful message about the future, sharing that the aforementioned doctor pulled down his video and apologized. As part of his update, the Tsu Lange Yor creative director added, “No hard feelings from my side whatsoever.”
