In the period leading up to Bruce Willis’ frontotemporal dementia diagnosis, the actor’s family began noticing that he no longer seemed like himself. FTD is a progressive, incurable brain disorder that can affect language, behavior and personality.
According to his wife, Emma Heming Willis, conversations with the 71-year-old actor increasingly felt disconnected. Willis, the proud Jersey native known for his “live it up” attitude, had also become noticeably quieter and more withdrawn at home.
Heming, 50, first questioned whether her husband was having trouble hearing. Willis had experienced partial hearing loss in his left ear after firing a gun without ear protection while filming the 1988 action classic Die Hard.
That made his March 2022 aphasia diagnosis especially shocking for his family and fans. Aphasia can impair a person’s ability to speak, understand language and communicate, a devastating development for an actor celebrated for playing fearless, determined characters.
The family announced Willis’ retirement from acting soon afterward, as his condition progressed and doctors explained that he would not return to the person he had once been.
Four years after his FTD diagnosis—and roughly a year since the Pulp Fiction star was last seen in public—a source told the Daily Mail that Willis “has declined a lot mentally” and is “living in his own world.” The source claimed the Hollywood icon is not fully aware of the extent of his cognitive decline.

“He knows his life has changed, but he doesn’t believe it has changed that much,” the source said.
“He rarely talks about it and does not complain. He can tell that something is wrong, but he does not completely understand what is happening.”
The source also described a significant shift in Willis’ personality. Once energetic and highly talkative, he is now “Mr. Mellow,” appearing calmer and more relaxed than in his younger years.
“Sometimes he looks like a baby studying something with curiosity, simply absorbing it,” the source said. “But physically, he still looks like a healthy middle-aged man.”
Long before doctors provided an explanation, the family had noticed that the man who once energized every gathering had stopped taking part in lively conversations at their weekly dinners. Those meals included his two young daughters with Heming Willis and his three adult daughters from his 12-year marriage to actress Demi Moore.
Doctors later identified the condition as primary progressive aphasia, a form of FTD that gradually damages the brain regions responsible for language and personality. Life expectancy is generally estimated at eight to 12 years after symptoms begin.
In February 2023, three months after Willis’ diagnosis was made public, his family thanked supporters for standing by the actor as he faced the debilitating illness. The disease is estimated to affect between 50,000 and 60,000 people in the United States.
“Your continued compassion, understanding and respect will help us give Bruce the fullest life possible,” the family said.
However, people who had worked with the Unbreakable star said they had detected warning signs well before his diagnosis was announced.
When Willis’ retirement was revealed after four decades in show business, nearly two dozen former colleagues told the Los Angeles Times that he had appeared to be experiencing serious cognitive difficulties for several years.
Although he made 25 movies between 2020 and 2023, many of them released directly to video, sources alleged that Willis sometimes struggled to remember his lines and worked with an actor who supplied dialogue through an earpiece.
The diagnosis stunned fans who had long associated Willis with the fearless, resilient heroes he played on screen

Some colleagues also questioned whether Willis fully understood what was happening around him on set. One alleged incident occurred during production of the 2020 film Hard Kill, when he reportedly fired a prop weapon before the correct cue. No one was injured, but sources said the moment unsettled the cast and crew.
Why, then, did it take so long for those around the actor to understand that he needed help?
The Mayo Clinic describes FTD as particularly difficult to diagnose. There is no single test that can identify it, while its symptoms may resemble other medical conditions, including liver or kidney disease.
Heming Willis experienced that uncertainty firsthand.
It took multiple examinations, consultations and an initially “vague” aphasia diagnosis before she understood what her husband was facing. Even then, receiving an explanation did not bring the relief she had hoped for.
Unlike Alzheimer’s disease, the most common form of dementia, FTD often does not begin with immediate memory loss. Instead, gradual changes in personality, judgment and language tend to emerge first and intensify over time.
FTD is the most common form of dementia among people under 60. In advanced stages, it can lead to problems similar to those seen in late-stage Alzheimer’s, including difficulty eating or swallowing, impaired mobility and increased vulnerability to infection.
FTD itself is not considered fatal, but pneumonia is the leading cause of death among people living with the disease.
There is currently no cure for FTD and no proven way to halt or prevent its progression. Certain medications and therapies may, however, help manage individual symptoms.
Heming has described learning that there was no treatment or cure as “one of the most traumatic experiences,” adding that she felt “completely alone.”

A source said Willis’ personality has changed considerably, describing him as calmer and more at ease than he was when he was younger
Heming soon discovered that she would not have to navigate the difficult reality alone. While caring for Bruce and their daughters, Mabel, 14, and Evelyn, 12, she received support from Moore and her three adult stepdaughters: Rumer, 38, Scout, 35, and Tallulah, 32.
Since their divorce, Moore and Willis have maintained a close friendship and an amicable co-parenting relationship. In her 2019 memoir, Inside Out, the Ghost star wrote that she was “very proud of our divorce,” despite how painful the separation initially was.
“It wasn’t easy at first,” Moore wrote. “But we found a way to move the heart of our relationship—the part that created our family—into something new, giving the girls a loving and supportive home with both parents.”

After Willis’ diagnosis, Moore became an important source of support, visiting him and sharing occasional updates with fans.
Speaking in 2024, Moore said there was “great beauty” in accepting Willis as he is now.
“When you hold on to what was, I think it’s a losing game,” she said at the Hamptons International Film Festival. Instead, she focused on sharing whatever time they had together for as long as possible.
Last year, Heming revealed that Willis had moved into a nearby single-story home where he receives care around the clock. She acknowledged that the arrangement had been emotionally difficult but said it ultimately helped the family.
“Ultimately, I could return to being his wife,” she told The Sunday Times. “And that is such a gift.”
The source now claims Heming is spending “a mega fortune” on her husband’s care, estimating the monthly cost at more than $30,000.
Once known for being the center of attention, Willis has adopted a much quieter life since his diagnosis
Heming has said that placing Willis in a nearby one-story home with 24-hour care was difficult, but ultimately helpful for the family
“It helps that he is worth a fortune, because insurance covers very little,” the source said.
A person close to the family told the Daily Mail last year that Heming was also dealing with the demands of managing Willis’ estate, estimated to be worth around $250 million.
“Caring for Bruce is not only about keeping him comfortable and helping him get through each day physically,” the source said. “It also means managing the considerable wealth he earned as one of Hollywood’s biggest stars.”
Because FTD can be isolating, Heming makes a point of ensuring Willis remains surrounded by people he knows, according to the insider. “There are always people with him whom he can interact with,” the source said.
The source described his home as “very bright and happy,” adding that “funny people” are often nearby to keep the atmosphere light.
“Bruce has family around him all the time,” the insider added. “They visit frequently.”
Tallulah is among his regular visitors. “She is amazing and truly loves her dad,” the source said.
His famous friends, however, are reportedly less present these days. “That is how the business works,” the source said. “Once you stop working, those glamorous friends disappear.
“In Hollywood, the A-listers are asking, ‘Bruce who?’”
Willis has largely avoided public view since his diagnosis. In a rare sighting last November, he was photographed walking by the coast in Los Angeles with a carer, holding the person’s hand for support.
His daily interests have also changed, though a source said he remains active and engaged rather than spending all day watching television.
“Bruce does not simply sit in front of the TV,” the source told the Daily Mail. “He gets excited about small things, like an ice cream cone or a car ride. He loves Neil Diamond. Music is wonderful for him.”
Before he became an actor, Willis pursued music for a short time, playing harmonica in blues bars to make a living during the late 1980s.

The source said Willis is also experiencing the speech difficulties that affected him when he was young.
As a child, Willis was nicknamed “Buck-Buck” because of his stutter. He learned to manage it after joining his school’s drama club.
When he struggled with a word, he would simply choose another one, the source explained. Willis even used that strategy when working with film scripts later in life.
Today, however, he is no longer able to control the stutter in the same way.
“He pauses for a long time, as if he is lost in thought,” the source said. “But he is really searching for the words.
“He is trying to work out how to express himself.”
The Daily Mail has contacted representatives for Willis for comment.
As Willis adapts to his new reality, his younger daughters are also coping with the loss of the father they once knew.
In an October 2025 interview with Vogue Australia, Heming said the girls “grieve” for their father because “he’s missing important milestones, and that’s tough for them.”
Although children can be resilient, Heming said she does not know whether “my kids will ever bounce back,” adding that they are learning to navigate the situation just as she is.
She has emphasized the importance of valuing every remaining moment of connection with Willis, knowing that those moments could disappear unexpectedly.
“Do I think he knows, ‘This is Emma, and we’ve been married for this many years?’” she told The Sunday Times. “I don’t know what that process is like for him.”
Still, she said there is something unmistakably familiar when he embraces her. “When he puts his arms around me, it just feels like Bruce,” she said. “That part is not different.
“And it is incredibly beautiful and incredibly heartbreaking.”
