Alfie Hewett was born with the congenital heart disorder Tetralogy of Fallot. When he was only 6 months old, he underwent surgery. He has Perthes disease, a rare hip condition that limits his ability to walk long distances.

The disorder Legg-Calvé-Perthes disease prevents blood flow from the pelvis to the hip joint. He has a significantly limited capacity to walk, and since he was six years old, he has been using a wheelchair. Hewett can walk, but he lacks traditional mobility and is unable to participate in able-bodied sports.

At the Cajun Classic in Baton Rouge, Louisiana, USA, in March 2018, Hewett captured his first Super Series singles championship. On September 2, 2018, at the US Open USTA Wheelchair Championships in St. Louis, he won his second Super Series match.
