Sean “Diddy” Combs was reportedly taken from federal prison early Saturday after suffering a head injury in a fall, according to a new report.
Sources at FCI Fort Dix in New Jersey told the publication that the 56-year-old rapper was transported by ambulance. He was reportedly “conscious and speaking” when he returned to the facility before sunrise Saturday.
Diddy’s spokesperson, Juda Engelmayer, told The Daily Mail: “We are aware of the reports concerning Sean Combs and are working to confirm the circumstances.
“Sean’s health and well-being are our immediate concern, and we appreciate everyone who has reached out.”
It remains unclear how Combs sustained the alleged injury, TMZ reports.
The Daily Mail has also contacted representatives for FCI Fort Dix for additional information.

Combs was sentenced last year to four years and two months in prison after being convicted on two prostitution-related charges following a highly publicized trial.
A jury found him guilty of two counts of transporting prostitutes across state lines for the purpose of sex. He was acquitted of the more serious charges of racketeering and sex trafficking.
His release date was initially set for May 2028, but it has been changed several times.
Last week, reports revealed that the rapper could spend additional time behind bars after his projected release date was pushed back.
Federal Bureau of Prisons records reviewed by the Daily Mail show that Combs is now scheduled to be released on March 2, 2028. That is six weeks later than his previous release date of January 21, 2028.
The timing of the change drew attention because it came slightly more than a week after reports said Combs had been returned to solitary confinement amid allegations that he received lavish “VIP” treatment in prison.
According to People, which first reported the revised release date, Combs remains in solitary confinement.
“For privacy, safety, and security reasons, we do not comment on the conditions of confinement for any incarcerated individual, including reasons for a change in release date,” the Bureau of Prisons said in a statement to the publication.
The latest development follows claims that Combs received special treatment while incarcerated.
The hip-hop mogul was reportedly moved back to solitary confinement after an NBC News report detailed allegations that inmates were doing tasks for him, including cooking, cleaning his cell and providing massages.
Sources told TMZ that the move was consistent with standard prison procedures while officials investigate the allegations.

However, it’s unclear if an investigation is still ongoing, and if his ongoing stay in solitary confinement is standard procedure.
The rapper was previously put in solitary back in July after getting into a fight with another inmate.
In the new NBC report, one inmate claimed Diddy has access to a cell phone where he receives nude photos from a former girlfriend as well as a ‘jail broken’ tablet that he’s used to watch his archnemesis 50 Cent’s damning documentary about him.
Diddy’s representative Juda S Engelmayer told the Daily Mail about the recent reports: ‘There is no way we can independently verify those rumors.’
Four men who spent time in Diddy’s unit opened up to the publication about his allegedly luxurious lifestyle – revealing how the music mogul has been living it up during his years-long prison sentence.
According to the insiders, Diddy enjoyed premium liquor smuggled in by guards, has purchased a mood-boosting antidepressant from inmates and has fellow prisoners store his fan mail and phone for him.
One inmate provides him with Chinese-style massages.
Maintaining such a lifestyle doesn’t come cheap; Diddy apparently splashes thousands of dollars a month to continue enjoying the luxury prison lifestyle.
The funding apparently comes from prisoners who work for Diddy, who have their commissary accounts replenished from people on the outside.

Photos obtained by the publication show the meals prepared by inmates specifically for him and others, pizza and a plate of meat with rice.
‘This is not some normal s*** you see in jail. This is some rich s***,’ according to one of the men.
In response to the article, Engelmayer told The Daily Mail: ‘The continued scrutiny of every mundane detail of Sean Combs’s life in prison has reached the point where the media characterizes ordinary prison life as news.’
‘The only real news is that Sean continues to serve his sentence within the rules and he patiently awaits the court’s decision on his appeal.’
