After appearing with Taylor Swift at the MTV VMAs and attending several events tied to her upcoming movie Verity, Dakota Johnson has become the subject of online speculation about possible plastic surgery.

Some social media users claim the actress “doesn’t look like the same person,” while others have suggested she may have had eyelid surgery because her eyes once appeared more hooded. One particularly detailed post alleged she had undergone a brow lift, upper blepharoplasty, hairline reduction or used wigs and extensions, along with buccal fat removal and liposuction beneath the chin and neck.

Other fans have pushed back against the cosmetic surgery rumors, arguing that Johnson’s hairstyle may be responsible for much of the change. “Dakota Johnson trimmed her bangs and immediately there’s plastic surgery allegations,” one person wrote.

Because most of those commenting are not medical professionals, their conclusions remain speculation. To offer a more informed perspective, what does a plastic surgeon make of the changes fans believe they see?

Dr. Omar Tillo, a plastic surgeon and medical director at ⁠CREO Clinic, told Showbiz Cheat Sheet that comparisons between older and newer photos do show several noticeable differences. “Her eyes appear more open, the upper part of the face looks slightly more lifted, and there also seems to be less fullness through the cheeks and lower face,” he said.

According to Dr. Tillo, the most apparent difference is around Johnson’s eyes. Older images show more upper-eyelid hooding, while recent photos appear to reveal more of the upper lid, giving her eyes a more open appearance. He explained that this can be consistent with an upper blepharoplasty, a procedure that removes a small amount of excess eyelid skin without dramatically altering the rest of the face.

Still, Dr. Tillo emphasized that photographs cannot establish whether Johnson actually had the procedure. He also noted that her face looks slimmer and more defined in some recent pictures, particularly around the cheeks and jaw. Buccal fat removal can produce that kind of contour by reducing volume in the lower cheeks, which may explain why some observers have drawn that comparison.

However, facial volume naturally changes with age, and even modest weight fluctuations can be especially visible on someone with a naturally slender face. For that reason, he said there is not enough evidence in photos alone to conclude that Johnson has undergone facial liposuction.

Could Dakota Johnson’s bangs be contributing to the way her appearance is being perceived?

Dr. Tillo said hair can significantly influence how a person’s facial proportions appear. Bangs reduce the visible height of the forehead and create a different frame around the eyes, making the upper portion of the face look shorter while emphasizing the eyes and cheekbones.

That means fans who believe Johnson’s bangs are part of the explanation may have a point. Dr. Tillo said the hairstyle could be intensifying the overall effect, although long-term photo comparisons appear to show changes involving the eyelids and facial fullness that cannot necessarily be explained by a haircut alone.

In his overall assessment, Dr. Tillo said that if Johnson has had cosmetic work, it may involve several subtle adjustments rather than one dramatic operation. Her eyes seem more open, her brows may appear slightly higher, and her cheeks and lower face look more defined. Together, even minor changes in those areas can noticeably affect the balance of the entire face.

At the same time, the surgeon stressed that the images being compared were taken years apart and under different circumstances. Makeup, changes in weight, aging, hairstyles, lighting, camera angles, lenses and facial expressions can all influence someone’s appearance. Without examining Johnson in person or reviewing her medical history, he said it is impossible to confirm whether she has had any procedures. At most, photos can suggest which cosmetic treatments might account for the differences people believe they see.
