Edward and Alphonse Elric paid a high cost for human transmutation in Fullmetal Alchemist. After Trisha, the mother of the two siblings, passed away, Edward promised his brother that they would bring her back to life with the power of alchemy. The siblings received training and developed a transmutation cycle to do so.
However, things went awry pretty quickly, and instead of bringing their mother back to life, they only received a grotesque mass of organs that could not even sustain itself. Additionally, Al lost his entire body, and Edward had to sacrifice his right arm on top of his severed leg to save Al’s soul.
Lelouch’s Joke Cost Him a Massacre in Code Geass
Lelouch could never have imagined that one slip of his powers would lead to a massacre in Code Geass. Lelouch used his Geass on his half-sister Euphemia by mistake, but his control unexpectedly skipped. All Lelouch did was joke to Euphemia about killing all the Japanese people.

Doma, the Upper Rank Two in Demon Slayer, is a brutal and powerful demon. For decades, Doma has killed and consumed humans, but not even in his wildest dreams could he imagine that eating a human would end up in his death. During the Infinity Castle Arc, Doma defeated and consumed Shinobu Kocho, the Insect Hashira, but he was not aware that Shinobu had spent over a year infusing her body with Wisteria poison.
Some time after Doma absorbed Shinobu, his body began to melt, and he became significantly less powerful. This provided Inosuke and Kanao with the perfect chance to behead the demon. Doma’s overconfidence and shortsightedness led to his eventual death.
Zeke Severely Dared to Underestimate Levi’s Ruthlessness In Attack on Titan
Zeke Yeager is a strategic mastermind in Attack on Titan, but he could not comprehend the bloodlust that hides in Levi. During the War for Paradis arc, Zeke transformed 30 Survey Corps soldiers into Pure Titans. Zeke used this opportunity to flee from the Corps and naively assumed that Levi would not slaughter his own subordinates to come after him.
Zeke’s assumption was proved wrong when Levi eliminated all the threats after only a brief moment of hesitation. Levi bombed the Beast Titan’s nape and pulled out Zeke. All For One, the ultimate villain of My Hero Academia, dug his own grave when he forcibly gave his younger brother a quirk. AFO did this out of a twisted sense of love and also to achieve dominance over his younger brother. However, AFO was unaware that his brother already had a quirk.
AFO forcefully implanted a quirk that he called Power Stock. It then merged with Yoichi’s pre-existing quirk to form a new quirk that is known as One For All. It is this quirk that was eventually inherited by Deku and led to AFO’s downfall.

Had The Reaper never trusted the apprentice, he would have continued with his assassination job without becoming a ticking bomb. It was the betrayal that set the events of Assassination Classroom in motion, which eventually led to Koro-sensei enlisting as Class 3-E’s teacher, only to train the students to kill him.

Solo Leveling’s Jinwoo made one fatal mistake, and it might be his biggest regret.
